This form is to be used to voluntarily opt out of Apple’s overdraft privilege. By opting out you agree to do so for a minimum of one year and must put any request to have the privilege reinstated in writing. Overdraft privilege opt out covers all overdraft transactions including checks, ACH, automated electronic debits, and debit card transactions.
